Black Creek Community Health Centre’s Yorkgate Mall location provides accessible, community-based health and wellness services with a focus on equity and addressing social determinants of health. Services include primary care delivered by physicians, nurse practitioners, and nurses; sexual and reproductive health services such as STI testing, birth control counselling, and pregnancy testing; chronic disease prevention and management supports including diabetes education; mental health counselling; exercise education/kinesiology; physiotherapy; dietitian services; foot care/chiropody; and harm reduction supports including safe-use supplies, and addiction supports. The site also offers health promotion, group programs, and community education for youth, families, and adults, along with referrals and system navigation support. The centre also hosts an Oral and Dental Health Clinic through Toronto Public Health. Many services are self referral, but some services require a referral. Language interpretation is available for those who require it. In-person and phone/virtual appointments are available.
